Welcome to Atlas Men's Health. These Terms and Conditions ("Terms") govern your use of our website at atlasmenshealth.com and the services provided by Atlas Men's Health, a trade name of PKNP Services LLC, with clinical services performed under PK NP in Family Health, PLLC and Atlas Wellness Adult Health NP, PLLC (collectively, "Atlas," "we," "us," or "our"), at our East Meadow, Midtown Manhattan, and telehealth locations. By accessing or using our website or services, you agree to be bound by these Terms. If you do not agree, please do not use our website or services.

The information provided on this website is for general informational and educational purposes only and does not constitute medical advice, diagnosis, or treatment. Content on this site should not be used as a substitute for professional medical advice from a licensed healthcare provider.

The services provided by Atlas Men's Health have not been evaluated by the Food and Drug Administration. Our treatments are not intended to diagnose, treat, cure, or prevent any disease. Individual results may vary. All treatments require a prescription and are administered under the supervision of a licensed healthcare provider.

2. Eligibility and Services

Our services are available to adults aged 18 and older. By using our services, you represent that you are at least 18 years of age and legally capable of entering into binding agreements.

Atlas Men's Health provides medical services including but not limited to: testosterone replacement therapy (TRT), peptide therapy, GLP-1 weight loss management, acoustic wave therapy (GAINSWave), PRP treatments (P-Shot), IV therapy, vitamin injections, botox/neurotoxins, and microneedling. Services are subject to medical evaluation and provider discretion. Not all services are appropriate for all patients.

3. No Doctor-Patient Relationship via Website

Use of this website, completion of online assessments (including the Low-T Quiz and Peptide Assessment), or submission of contact forms does not establish a doctor-patient relationship. A provider-patient relationship is established only when you have been formally accepted as a patient and have received a clinical evaluation by one of our licensed providers.

4. Telehealth Consent

Atlas offers telehealth services to patients in New York, New Jersey, Virginia, Florida, Nevada, and Iowa. By using our telehealth services, you acknowledge and agree that:

• Telehealth involves electronic communication between you and your provider and is not the same as an in-person visit
• There are potential risks including technology failures, interruptions, and limitations in the ability to perform physical examinations
• Your provider may determine that telehealth is not appropriate for your condition and may require an in-person evaluation
• You have the right to withdraw consent for telehealth at any time
• Telehealth consultations are conducted through HIPAA-compliant platforms

5. AI-Assisted Documentation and Recording Consent

Atlas Men's Health uses technology to enhance the accuracy and efficiency of your care:

• AI Medical Transcription: We use HEIDI, a HIPAA-compliant AI transcription platform, during in-person visits and telephone consultations to generate clinical documentation. All AI-generated notes are reviewed and approved by your licensed provider before being finalized. AI transcription is a documentation aid only and does not make clinical decisions.
• Call Recording: Telephone calls to and from our offices may be recorded through our Zenoti practice management system for quality assurance, training, appointment verification, and compliance purposes.
• Opt-Out Right: You may request that AI transcription be turned off during any visit by informing your provider. Opting out will not affect the quality of your care or your eligibility for services. Your provider will take notes manually instead.

By receiving services at Atlas, you acknowledge and consent to the use of AI-assisted documentation and call recording as described above and in our Privacy Policy.

6. Payment, Billing, and Insurance

Atlas Men's Health operates as an out-of-pocket, direct-care facility. We do not accept or bill insurance directly. Key financial terms:

• Payment is due at the time of service unless you are enrolled in a membership program with defined payment terms
• We accept major credit/debit cards, HSA/FSA cards, and cash
• Financing is available through Affirm, Klarna, and CareCredit
• Superbills are available upon request for you to submit to your insurance for potential out-of-network reimbursement; reimbursement is not guaranteed
• Laboratory services may be billed to your insurance through third-party labs
• Membership fees, once processed, are subject to the cancellation terms outlined in your membership agreement
• Atlas reserves the right to modify pricing with reasonable notice

7. Cancellation and No-Show Policy

We request at least 24 hours' notice for appointment cancellations or rescheduling. Repeated no-shows or late cancellations may result in fees or limitations on future scheduling. Specific cancellation terms for membership programs are outlined in your membership agreement.

8. Informed Consent for Treatment

All medical treatments at Atlas require informed consent. Before beginning any treatment, your provider will explain the nature of the procedure, expected benefits, potential risks and side effects, alternative treatment options, and the consequences of choosing not to proceed. You will be asked to sign informed consent documentation as required.

9. Results Disclaimer

Individual results may vary. Testimonials, before-and-after imagery, and outcome descriptions on this website reflect individual patient experiences and are not guarantees of results. The effectiveness of any treatment depends on many factors including your individual health, adherence to treatment protocols, lifestyle choices, and biological response.

10. Intellectual Property

All content on this website — including text, images, graphics, logos, videos, assessments, calculators, and design elements — is the property of Atlas Men's Health or its licensors and is protected by copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content without our prior written consent.

11. Website Accuracy

We make reasonable efforts to ensure the information on our website is accurate and up-to-date. However, we do not warrant that all information is error-free, complete, or current. Service descriptions, pricing, and availability are subject to change without notice. In the event of a discrepancy, the information provided during your clinical consultation takes precedence over website content.

Our website may contain links to third-party websites and services (including payment processors, patient portals, pharmacy services, and social media platforms). These links are provided for convenience only. Atlas does not endorse, control, or assume responsibility for the content, privacy policies, or practices of any third-party site.

13. Limitation of Liability

To the fullest extent permitted by applicable law, Atlas Men's Health, its owners, officers, employees, and providers shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of this website or our services. This includes, without limitation, damages for loss of data, loss of revenue, or personal injury, except where caused by gross negligence or willful misconduct.

Our total liability for any claim arising from website use shall not exceed the amount you paid to Atlas in the twelve (12) months preceding the claim.

14. Indemnification

You agree to indemnify, defend, and hold harmless Atlas Men's Health, its owners, officers, employees, and providers from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorney's fees) arising out of or related to your violation of these Terms, your misuse of our website or services, or your violation of any law or the rights of any third party.

15. Governing Law and Dispute Resolution

These Terms are governed by and construed in accordance with the laws of the State of New York, without regard to conflict of law principles. Any dispute arising from these Terms or your use of our services shall be resolved through binding arbitration administered in Nassau County, New York, in accordance with the rules of the American Arbitration Association, unless injunctive relief is sought. You agree to waive any right to a jury trial.

16. SMS Terms and Conditions

By opting in to SMS communications from Atlas Men's Health, you agree to receive text messages related to appointment confirmations, reminders, prescription alerts, and service updates.

• Message frequency varies; typically 4–8 messages per month
• Message and data rates may apply depending on your carrier
• Text STOP to cancel at any time; text HELP for assistance
• Consent is not a condition of purchase or treatment
• Mobile information and SMS opt-in data will not be shared with third parties for marketing or promotional purposes
• Carriers are not liable for delayed or undelivered messages

17. Severability

If any provision of these Terms is held to be unenforceable or invalid, that provision shall be modified to the minimum extent necessary to make it enforceable, and the remaining provisions shall continue in full force and effect.
