Informed Consent

This page provides general information about informed consent at Atlas Men's Health, a trade name of PKNP Services LLC, with clinical services performed under PK NP in Family Health, PLLC and Atlas Wellness Adult Health NP, PLLC. Specific consent forms are provided and signed in-office or via our patient portal prior to each treatment.
What Is Informed Consent?
Informed consent is the process by which your healthcare provider explains a proposed treatment or procedure, including its benefits, risks, alternatives, and potential outcomes, so that you can make an educated decision about your care. At Atlas Men's Health, we believe informed patients make the best decisions about their health.
General Consent for Treatment
By receiving services at Atlas Men's Health, you acknowledge and consent to the following:
- You authorize your provider and their clinical staff to perform examinations, diagnostic procedures, laboratory testing, and treatments as deemed medically appropriate
- You understand that medicine is not an exact science and that no guarantees have been made regarding the outcome of any treatment
- You understand that all treatments carry potential risks, including but not limited to: infection, bleeding, bruising, allergic reaction, pain, scarring, nerve injury, and treatment failure
- You have been given the opportunity to ask questions and have them answered to your satisfaction
- You have disclosed your complete medical history, current medications, allergies, and any conditions that may affect treatment
- You agree to follow your provider's post-treatment instructions and to communicate any adverse reactions promptly
Service-Specific Consent
In addition to general consent, the following services require separate, specific informed consent documentation that will be provided before treatment begins:
Testosterone Replacement Therapy (TRT)
Consent covers the nature of testosterone therapy, method of administration (injection, topical), expected benefits, potential side effects (including effects on fertility, blood counts, cardiovascular risks, mood changes), monitoring requirements (regular bloodwork), and the ongoing nature of therapy.
Peptide Therapy
Consent covers the specific peptide(s) prescribed, method of administration, expected timeline for results, potential side effects, off-label use where applicable, and the compounding pharmacy source.
GLP-1 Weight Loss Therapy
Consent covers the medication prescribed (semaglutide or tirzepatide), dose titration schedule, potential gastrointestinal and other side effects, the importance of lifestyle modifications, monitoring requirements, and conditions that may contraindicate use.
GAINSWave Acoustic Wave Therapy
Consent covers the nature of low-intensity acoustic wave therapy, expected number of sessions, potential side effects (mild discomfort, temporary redness), expected outcomes and timelines, and the distinction from surgical interventions.
P-Shot (Platelet-Rich Plasma)
Consent covers the blood draw and PRP preparation process, the injection procedure, potential risks (bruising, swelling, infection, temporary discomfort), expected outcomes and recovery, and that PRP uses your own blood products.
IV Therapy and Vitamin Injections
Consent covers the specific formulation being administered, potential risks of intravenous or intramuscular injection (bruising, infiltration, phlebitis, allergic reaction), and contraindications based on your health history.
Botox / Neurotoxins
Consent covers the specific product used, injection sites, potential side effects (bruising, headache, temporary weakness, drooping, allergic reaction), expected duration of results, and that repeat treatments are necessary to maintain results.
Microneedling (with or without PRP)
Consent covers the procedure, expected downtime (redness, mild swelling for 24–72 hours), potential risks (infection, scarring, hyperpigmentation), pre- and post-care instructions, and the number of sessions typically recommended.
AI-Assisted Documentation Consent
Atlas Men's Health uses HEIDI, a HIPAA-compliant AI-powered medical transcription platform, to assist with clinical documentation. By receiving services, you acknowledge and consent to the following:
- HEIDI may listen to and transcribe your conversations with your provider during in-person visits and telephone consultations
- Audio is processed in real-time to generate clinical notes — it is not permanently stored by the transcription service after notes are generated
- All AI-generated clinical notes are reviewed, edited, and approved by your licensed healthcare provider before being added to your medical record
- HEIDI is a documentation tool only — it does not make clinical decisions, diagnoses, or treatment recommendations
- You have the right to opt out of AI-assisted transcription at any time by informing your provider at the start of your visit; your provider will document notes manually instead
- Opting out will not affect the quality of your care or eligibility for any services
Call Recording Consent
Atlas Men's Health uses the Zenoti practice management platform, which may record telephone calls to and from our offices. By calling or receiving calls from Atlas, you acknowledge and consent to the following:
- Calls may be recorded for quality assurance, staff training, appointment verification, and compliance purposes
- Recordings are stored securely within the Zenoti platform and are subject to the same privacy protections as your other health information
- Recordings are accessed only by authorized Atlas staff on a need-to-know basis
- Recordings are retained in accordance with our data retention policies and applicable law
Telehealth Consent
If you receive care via telehealth, additional consent is required acknowledging:
- The nature and limitations of telehealth compared to in-person evaluation
- Potential risks related to technology (interruptions, security, inability to perform physical exam)
- Your right to refuse or withdraw consent for telehealth at any time
- That your provider may determine an in-person visit is necessary
- The telehealth platform used is HIPAA-compliant
- Laws and regulations regarding telehealth in your state of residence
Consent for Minors
Atlas Men's Health does not provide services to individuals under the age of 18. All patients must be legal adults.
Withdrawal of Consent
You have the right to withdraw consent for any treatment at any time, including before or during a procedure. Withdrawal of consent will not affect your ability to receive other services or your relationship with Atlas Men's Health. If you withdraw consent during an ongoing treatment program (such as TRT or GLP-1 therapy), your provider will discuss safe discontinuation protocols.
Photographic Consent
With your separate written permission, before-and-after photographs may be taken for your medical record and, if authorized, for educational or marketing purposes. Photographic consent is always optional and never a condition of treatment. You may revoke photographic consent at any time.
Financial Consent
By receiving services, you acknowledge that:
- Atlas Men's Health is an out-of-pocket, direct-care facility and does not bill insurance directly
- Payment is due at the time of service unless enrolled in a membership program
- You are responsible for any applicable fees, including those for missed appointments or late cancellations
- Superbills are available upon request for potential insurance reimbursement, which is not guaranteed
